What does your nervous system actually do?
Most people think of the nervous system as the brain plus a network of wires that carry pain signals. It is much more than that. Your nervous system is part of how your body senses, protects, regulates and adapts, every second of the day.
A large part of that work is done by the autonomic nervous system, the automatic branch you never have to think about. It has two sides. The fight or flight side gears you up: heart rate rises, muscles tense, digestion slows, senses sharpen. The rest and digest side does the opposite: it settles your heart rate, supports digestion, allows deep sleep and lets the body get on with repair and recovery.
Both sides are useful. You need the alarm system when you step off a kerb into traffic, and you need the recovery system every night. Health depends on the ability to move flexibly between the two, in the right amounts, at the right times.
What is nervous system dysregulation?
Nervous system dysregulation is what happens when that flexibility is lost. The system becomes stuck with the alarm turned up and the recovery response turned down, so the body keeps behaving as if it is under threat even when nothing dangerous is happening.
This is not a character flaw and it is not something you are imagining. The nervous system is doing exactly what it is designed to do, which is to protect you. The problem is that once it has been running in a protective state for months or years, that protection stops being useful. A system stuck on high alert has less capacity for sleep, digestion, repair, concentration and calm, because those are exactly the functions it turns down when it believes you are in danger.
This does not mean your symptoms are in your head. It means the system that senses and protects you may be part of the picture.

Common signs your nervous system may be stuck on high alert
No single sign proves anything on its own. It is the cluster, and the way symptoms behave, that tells the story. You may recognise yourself in several of these:
- Pain that persists or keeps flaring, often out of proportion to what you did that day, or in more than one area of the body.
- Feeling tired but wired: exhausted all day, then unable to switch off at night.
- Unrefreshing sleep, waking through the night, or waking as tired as you went to bed.
- Ongoing fatigue and energy crashes, especially after doing more than usual.
- A sensitive digestive system: bloating, cramping or IBS type symptoms that flare in stressful periods.
- Feeling on edge: a racing heart, jumpiness, or a low hum of anxiety in the body even when nothing is wrong.
- Brain fog and poor concentration, finding it hard to focus or hold information.
- Cold hands and feet or a general sense that your circulation is sluggish.
- Symptoms that track your life load: worse in stressful weeks, after poor sleep or during illness, better on holiday.
Two things in that list matter more than any single symptom. The first is persistence: any symptom that keeps recurring, or simply will not resolve month after month, suggests a layer of nervous system involvement, because a symptom cannot persist unless the system that senses and protects you stays sensitised to it. The second is timing: when symptoms rise and fall with stress, sleep, exertion or digestion, that pattern suggests a regulation layer is involved, not just a local tissue problem.
Why one system can sit under so many different symptoms
It can seem implausible that pain, fatigue, gut symptoms, sleep and mood could share anything at all. They feel like separate problems, and they are usually treated as separate problems. But each of them depends on a nervous system that can regulate itself.
Persistent pain. Pain is always created by the brain, as the output of a system whose job is to protect you. When the nervous system has got stuck trying to protect you, pain can persist or spread even when tissues have healed. Research consistently finds that the automatic, protective side of the nervous system is frequently altered in people with persistent pain.
Fatigue. Recovery is an active process that happens when the rest and digest system is allowed to work. A body held on alert around the clock spends its energy on vigilance rather than repair. Research in chronic fatigue has found altered automatic regulation of the heart compared with healthy people.
Sleep. Falling asleep, and reaching deep sleep, requires the nervous system to shift out of its alert state. Heart rate variability, which is a practical window onto how much rest and digest activity a person has, is closely tied to sleep and recovery.
Gut symptoms. Your gut and your brain are in constant two way conversation. IBS type symptoms are not only a gut problem; brain to gut signalling and the automatic nervous system can be a big part of the picture.
Feeling anxious or on edge. Anxiety is partly a body state. A system stuck in fight or flight produces the racing heart, tight chest and restlessness that many people experience as anxiety, which is one reason calming the body can matter as much as calming the mind.
Inflammation and immune activity. The nervous system and the immune system talk to each other constantly, particularly through the vagus nerve, which has recognised anti-inflammatory signalling roles. This is an active research area, and it is one more reason the nervous system deserves a place in the conversation about persistent, whole body symptoms.

To be clear about what we are not saying: the nervous system is not the only cause of these conditions, it is a layer, and in some people it is the layer that has been keeping everything else stuck.
How do you know if your nervous system is involved?
Ask two questions about your symptoms.
The first is: how long has this been going on? Persistence is itself the clue. A short lived symptom can usually be explained by local irritation, but a symptom that keeps recurring, or has been present for months or years, is being actively maintained by something. The nervous system, whose whole job is to sense, protect and regulate, is very often part of that maintaining layer. This is why the latest research keeps finding nervous system and autonomic involvement across chronic pain, chronic fatigue type presentations, long COVID type presentations and conditions like chronic headaches or migraines.
The second is: what changes them? Look for symptoms that shift with load, stress, sleep, position, exertion, digestion or recovery state. A knee that hurts on every tenth step is behaving like a local, mechanical problem. Pain, fatigue or gut symptoms that surge in stressful weeks, settle on holiday, and flare after broken nights are behaving like a system under load.
Some presentations show this especially clearly. In people with ongoing symptoms after COVID, formal testing found problems with the automatic control of heart rate and blood pressure in 23 of 24 patients assessed in one study, including racing heart on standing and blood pressure changes. Not everyone with long COVID type symptoms has this pattern, but it shows how real, measurable and physical nervous system dysregulation can be, even when standard tests look normal.
